Azzurro Health Wellness Center Terms & Conditions
1. Appointment Policy
1.1. All appointments must be booked in advance, either online, by phone, or in person.
1.2. Clients are requested to arrive at least 10 minutes prior to their appointment.
1.3. Late arrivals may result in a shortened session, and full fees will still apply.
2. Cancellation and Rescheduling Policy
2.1. Cancellations or rescheduling must be made at least 24 hours prior to the appointment time.
2.2. Cancellations made with less than 24 hours’ notice may incur a cancellation fee equal to 50% of the session cost.
2.3. No-shows will be charged the full session fee.
3. Payment Policy
3.1. Payment is due at the time of service unless prior arrangements have been made.
3.2. Azzurro Health accepts cash, credit/debit cards, and electronic transfers.
3.3. Outstanding balances must be cleared before future appointments are scheduled.
4. Confidentiality and Privacy
4.1. All client information is kept confidential and complies with the applicable data protection laws.
4.2. Azzurro Health will not share personal information without prior consent, except as required by law.
4.3. Clients have the right to access their personal records upon written request.
5. Health and Safety
5.1. Clients must disclose any relevant medical history, current conditions, and medications prior to receiving treatment.
5.2. Azzurro Health reserves the right to refuse treatment if it is deemed unsafe for the client.
5.3. Clients exhibiting symptoms of contagious illnesses are kindly asked to reschedule their appointments.
6. Medical Disclaimer
6.1. Services provided by Azzurro Health are complementary and should not replace medical advice from licensed healthcare providers.
6.2. Clients are responsible for consulting their primary healthcare provider before undergoing any treatment.
7. Liability
7.1. Azzurro Health and its practitioners are not liable for any injury, loss, or damage that may arise from treatments unless proven negligent.
7.2. Clients assume all risks associated with the treatments and acknowledge that results may vary.
8. Termination of Services
8.1. Azzurro Health reserves the right to refuse or discontinue services to clients who display inappropriate behavior or fail to adhere to the practice’s policies.
9. Feedback and Complaints
9.1. Azzurro Health welcomes feedback to improve services.
9.2. Complaints should be directed to the practice manager and will be handled promptly and confidentially.
10. Amendments
10.1. Azzurro Health reserves the right to update these Terms and Conditions at any time.
10.2. Clients will be notified of any significant changes via email or in-practice announcements.
